This Online Form Submission is for customers seeking warranty service for products BUILT BY Ocosmo. We will require all of the following information on this form to process your warranty service request.

An RMA # will be issued within 48 hours by email. Please read the Shipping Instruction below carefully.

Shipping Instructions

  • 1.FORM

    Completely fill out the Return Merchandise Authorization (RMA) form. Incomplete RMA request will be refused.
    An RMA number will be issued within 48 hours and is valid for 15 days.
    No PO Box address will be accepted.
  • 2.PACK

    To avoid damages, please ship the product(s) with the original packaging materials. Ocosmo will NOT be responsible for any damages or lost items.
    Send Monitor/TV only. Do not send in Cables, Accessories, Remotes, or Stand/Base. Please cover product(s) with a plastic bag or protector to prevent screen damage from the packaging material and foam.
  • 3.SHIP

    Postage, insurance, and/or Shipping costs incurred in returning equipment for warranty service are your responsibillity.
    Ocosmo will not responsible for scratches or damages. Clearly print the RMA number on the outside of the shipping package and ship it to: 16800 E.     The RMA number is only valid for 15 days and must be clearly displayed on the outside of the shipping package. If the RMA form is incomplete or the return shipment does not have an RMA number clearly visible on the outside of the shipping label, the product will be refused and returned at the customer's expense.
